Preventive Membership Plan

  • Professional cleanings (2/year)
  • Doctor exams (1-2/year)
  • Bite wing X-rays (as prescribed by doctor)
  • Emergency exam, if needed (1/year)
  • Fluoride treatment (as recommended)
  • 20% off ANY treatment you require throughout the year

*For patients with regular cleanings and absence of periodontal disease.

$456 per adult ($38/month)

$348 per child ($29/month)

First 3 months due upon activation, billing begins on 4th month.

Perio Membership Plan

Only $57/month

Are you prone to more dental issues or have had gum disease in the past? The Perio Plan is perfect for your unique needs.

100% Covered: 

  • Periodontal maintenance cleanings (3-4/year)
  • Doctor exams (1-2/year)
  • Bite wing X-rays (as prescribed by doctor)
  • Emergency exam, if needed (1/year)
  • Fluoride treatment (as recommended)
  • 20% off ANY treatment you require throughout the year
  • 15% off cosmetic treatment

*For patients requiring periodontal maintenance due to prior treatment of periodontal (gum) disease.

Q: What forms of payment are accepted?
A: All forms of credit and debit cards including Visa, Mastercard, Discover and American Express.

Q: When can I start using my membership? 
A: Your membership is effective immediately!

Q: How much do I pay to sign up for my club membership?
A: The first 3 months will be due to activate your membership plan. You’ll only have to pay this advance upon activating your plan. Thereafter, monthly payments will be due starting the 4th month of membership.

Q: Can I prepay my membership fees?
A: Absolutely! By paying the full annual membership you can take advantage of a 5% discount!

Q: How do I schedule my preventive care? 
A: Simply call our office and setup an appointment for yourself or your family member.

Q: How can I use my Member Exclusive discount for restorative, elective or cosmetic treatment?
A: Simply schedule an appointment with our office and be sure to tell us you’re a member. When you arrive, we’ll make sure your membership is “current” and you will be eligible for instant savings off our regular fees.

Q: How do I know if I need the Perio Membership? 
A: If you’re a current patient, please contact us to confirm you’re on Perio Maintenance. If you are a new patient, we will make a recommendation after your intial exam.

Q: What happens if I miss an appointment? 
A: It will be your responsibility to reschedule. Please see our office policy regarding no-shows and last minute cancellations.

Q: Can I add members to my account?
A: Absolutely! Your entire family can join.

Q: Can I transfer my membership to another dentist? 
A: Your club membership is solely with our office and as such is non-transferable. If you move to another area, you may cancel your membership at any time.

Q: How can I check on the status of my account? 
A: Contact our office and we can verify the status of your account for you.

Q: Can I change or update my billing information?
A: Yes! Simply contact our office staff and we can update any changes.

Q: How do I cancel my membership? 
A: Contact our office team and they can cancel your membership. Cancellations are accepted at any time and no future payments will be due.

Q: What happens if my payment doesn’t go through? 
A: We will contact you if your credit card is not able to process your payment due. It is our policy to attempt to re-process the payment due a minimum of once. If we do not hear back from you within 28 days of original processes time, your account will be canceled.

Q: If my account is canceled can I reactivate my membership?
A: Yes, you can reactive your account at any time. The first 3 months will be due at this time.

Q: If I cancel, can I get a refund on my membership?
A: No. Refunds will not be provided for dues paid under any circumstances, including failure to schedule and maintain appointments.

Q: What information is required for me to sign up for my club membership?
A: An email address for receipts, credit or debit card, full name, address, phone and date of birth.
